A child who complains of their neck worries more than an adult doing the same, and that is right: the things to rule out are few, but they must be ruled out quickly. The rest, in the great majority of cases, resolves on its own.

⚠ Important: read this before continuing

The exercises and advice in this article are general indications and may not suit your situation. Every person has a unique clinical history: what works for others could be counterproductive for you.

Want a professional's opinion? Start from the questionnaire: you register and see who will follow you, a physiotherapist on the Italian professional register chosen for your case, whom you write to in chat. If you would rather hear their voice, the first consultation is free and we look at your situation together.

First of all: what needs seeing straight away

This list comes before any reasoning about causes.

  • Fever together with stiffness of the neck, especially if the chin does not reach the chest. It is the combination that suggests an infection of the meninges and is not to be waited on: emergency department.
  • After a trauma - a fall, a sports collision, a car accident - a child's neck must be assessed, always.
  • Weakness, tingling or loss of strength in one arm.
  • Head held tilted that does not straighten, especially if it appeared suddenly and comes with difficulty swallowing or speaking.
  • Pain that wakes the child at night and does not change with position, or that comes with weight loss.
  • Swelling or a palpable lump in the neck that does not go down within a few days.

Outside these cases, a child's neck pain is almost always benign.

The causes, stated for what they are

The number one suspect is the only one on which we have clear data: sleep and held positions. A neck that locks on waking or after hours in the same position is the most common story of all, and it passes within days.

Screens. Here it is worth being honest, because it is the explanation everyone takes for granted. For screen time in general, the association with neck pain is not statistically significant: odds ratio 1.13, 95% confidence interval 0.98 to 1.30. And almost all the available studies are cross-sectional, that is, they photograph children at one moment: they do not say whether the screen produced the pain or the pain changed the way the child sits. The full picture, with the numbers for each type of device, is in children, screens and the neck: how much posture really matters.

This does not mean screens have nothing to do with it. It means the right target is not correct posture - which no child keeps anyway - but how long they stay still: where the tablet rests, how often they get up, how much they move during the rest of the day.

The backpack. The link with neck pain is weaker than common sense suggests, and above all a backpack does not cause spinal deformity. Adjusting the straps so that it stays high and close is still a good idea, but it is not the main lever.

Stress. In children, distress often passes through the body before it passes through words. A difficult period at school, a move, a change in the family can appear as neck tension or headache. It is worth keeping among the hypotheses without making it the only one.

An important point: before trying these exercises, make sure you have had a correct diagnosis. People often think they have one problem, but the real cause of the pain is another. Doing the wrong exercises can make the situation worse instead of better.

How it presents

An older child says it: it hurts here, I cannot turn my head this way. A younger one often does not, and it must be read differently - they keep the head tilted always to the same side, avoid turning and rotate the whole trunk, become irritable, do not want to be held in a certain way.

Headache that starts at the back of the neck and climbs is common and usually follows the neck pain rather than preceding it.

What to do in the first few days

Get the neck moving, do not immobilise it. This is the thing that changes timing the most, and it is counterintuitive: movement within the pain, without forcing, shortens recovery. A collar is not needed and lengthens recovery.

Keep the day normal. School, play, sport if the pain allows. Taking everything away out of caution tells the child that their neck is fragile, and that is an idea that later takes effort to dismantle.

Heat if it brings relief. A warm pack for ten minutes works on the symptom, not on the timing, but if it makes them feel better, that is perfectly fine.

Watch the still hours, not the posture. A rule that works: every half hour in front of a screen, get up and do something standing for two minutes. This changes more than any correction of position.

If it does not pass

A week is a reasonable checkpoint. If the pain is the same, if it always appears in the same spot, if it limits sport or if it returns cyclically, an assessment helps to understand where it comes from - and in most cases to reassure with more precision than an article can.

Ri-Hub offers online physiotherapy sessions from 30 minutes at 29,90 €, and the first 20-minute consultation is free. For minors, the consent of a parent is required.

Domande frequenti

When is a child's neck pain urgent?

When there is fever together with stiffness of the neck, especially if the chin cannot reach the chest: it is the combination that must be seen right away, without waiting. Likewise after a trauma, in the presence of weakness or tingling in an arm, if the child holds the head tilted and cannot straighten it, or if the pain wakes them at night and does not change with position.

Is it the fault of tablets and smartphones?

Less than is said. For screen time in general, the association with neck pain is not statistically significant (odds ratio 1.13, interval 0.98 to 1.30), and the studies are almost all cross-sectional: they photograph a moment and do not say what came first. What really matters is not the right posture, it is where the screen rests and how often the child gets up.

Does a heavy backpack hurt the neck?

The link is less solid than common sense suggests, and above all a backpack does not cause spinal deformity. Adjusting the straps so that the bag sits high and close is still sensible: it costs nothing and stops the child walking off balance.

What can I do at home in the first few days?

Get the neck moving within the pain rather than immobilising it, keep the day normal, and use heat if it brings relief. No collar, which lengthens recovery. If nothing changes in a week, an assessment is worthwhile.

Could it be stress?

Yes, and in children it often shows up in the body before it shows up in words: a difficult period at school or a change in the family can appear as neck tension. It is worth asking, without making it the only explanation.

Dal singolo esercizio al percorso

Questi esercizi sono generici. Il tuo no.
